Or someone taking off with the gear pins still in. Has happened at my company and other airlines before.

Or not putting the nosewheel steering lock pin in. At least there's no way you could take-off that way but the chance of running into something / someone is bad enough. (I did once 😳, but ONLY once and luckily, it only resulted in embarrassment on my part).

There was a guy at CVG a couple years ago that was in a hurry and didn’t wait for a longer interphone cord, then tripped and got run over by 747 nose tire during pushback. Lost both his legs but survived. 

Well, I suppose if you're going to get your legs run over, might as well have it done by the 500,000 pound Cargo jet instead of some schmuck's Mooney . . . . .
